If you decide to receive services at Clarkson Counseling, there is some paperwork that you will need to complete. In order to save you some time, we have made the paperwork available online. The form can be completed online and printed (Word Document) or printed and completed by hand (PDF Document). Just click on the link below.

If you have difficulty downloading the file, don't worry about it, since it is not necessary for this paperwork to be completed prior to arriving for your first session. You will be given an opportunity to complete it at the office.
